Forms

July 2013:  Until further notice, applications for the Master of Science in Health Care Policy and Management have been suspended.

The Advanced Certificate in Health Care Management has an application deadline of November 1, 2013, for Spring 2014 start.  Students who are accepted must be matriculated for at least two semesters.

Non-matriculated Information:  You must be a United States citizen, have permanent resident status, or hold an H1 or H4 visa in order to take classes as a non-matric. You must submit a copy of your visa status or permanent resident status (copy both sides) to Fran Shaw.

If you hold an F1, B1, or B2 visa, you CANNOT apply as a non-matric.

Non-matrics can take a maximum of 12 credits total and no more than 6 credits in any given semester.   Non-matric forms due for consideration must be received by the due date:

Spring Semester - January 9
Summer Session 1 - May 14
Summer Session 2 - June 25
Summer Session Z - May 14
Fall Semester - August 12 

Forms for Non-matric Applicants

You must complete (1) the non-matric form, (2) the health form for part-time students, and (3) sign off on the meningitis form.

 (1) The completed non-matric form should be returned to Fran Shaw (address is on the form).  

  The non-matric application is set up as a form.  Please click in the first box, type information, and then press tab to go to the next box.

 (2) The completed health form for part-time students should be returned to Student Health (address is on the form).

(3) Once you have your Stony Brook ID number, log on to SOLAR.  Check your "To Do List".  You must sign off on the meningitis form.

 

Forms for Matriculated Students

You must complete (1) non-clinical programs student health form and (2) sign off on the meningitis form.

 (1)  Students who have been accepted for matriculation into the Certificate program must go to this web site:  http://www.stonybrook.edu/hscstudents/new_students.shtml

Look under School of Health Technology and Management.  Download the Non-clinical Programs Student Health form Complete this form and mail it back to STUDENT HEALTH (address is on the form).   Keep a copy for your records.

(2) Once you have your Stony Brook ID number, log on to SOLAR.  Check your "To Do List".  You must sign off on the meningitis form.
